Our team will walk alongside you, answer questions, and help you gather everything you need for a smooth move-in. Let us help!

What You'll Need

When preparing to move in, please bring the following documents with you so we can ensure all your health and financial needs are met:

  • Social Security Card 
  • Insurance Cards 
  • Medicare and/or Medicaid Cards 
  • Power of Attorney Documentation (both healthcare & financial)

  • Advance Healthcare Directives (Living Will, POLST forms, etc.)

We'll Guide You Through the Details

  • Review paperwork together and answer your questions 
  • Help with Medicaid applications, if needed 
  • Coordinate with your doctors, hospitals, or care providers
